Does this sound like you?
Different seasons. The same need: someone who actually listens.
I want to get pregnant, and nobody will tell me straight whether I can stay on my medication.
Pregnancy & postpartumI had my baby and I don't feel like myself. Everyone says it's normal. It doesn't feel normal.
Pregnancy & postpartumI lost a pregnancy. Everyone checked on the baby. Nobody checked on me.
Pregnancy loss & birth traumaI've been “high-functioning” my whole life. I'm exhausted. Could this actually be ADHD?
ADHD in womenThe week before my period I'm a different person, and I keep being told it's “just PMS.”
PMDD & reproductive psychiatryI'm not in crisis. I'm just running on empty, and I don't know how much longer I can keep this up.

How care works here
Slow enough to get it right. Steady enough to feel the difference.
No ten-minute visits. No one-size-fits-all plan. Here's what working with R3 actually looks like.
-
An unhurried first visit
Your evaluation is 60 to 90 minutes. We hear the whole story: your history, your symptoms, what's already been tried, and what's going on in your body and your life right now.
-
The whole picture, not just a prescription
Labs and medical rule-outs when they're warranted. Hormones, sleep, nutrition, stress. Then a plan you helped build: medication if it's right for you, integrative support, or both.
-
Steady follow-up until you feel like you
We start low and go slow. Monthly visits while we fine-tune, then every three months once you're stable, by video from home or in person in Clayton.
What makes R3 different
We don't throw medication at symptoms. We find out what's going on.
A new mom came to us with anxiety and insomnia she'd never had before. She'd been given medication after medication, and nothing worked. We ran labs first. It was her thyroid.
That's what “whole picture” means here

Meet your provider
Hi, I'm Tiirini.
I'm a board-certified psychiatric nurse practitioner, a psychiatric nurse since 2015, and a mom of two. I specialize in women's mental health through pregnancy, postpartum, and the seasons that follow, with an integrative lens that looks at the body as well as the mind.
This practice is personal. After my own miscarriages and an ectopic pregnancy that ended in emergency surgery, no one checked on me. Not once. I saw the gap in care up close, and I built R3 so no woman in my care would go unseen like that.
I'm a Black woman, a millennial, and a woman of faith. For many of my patients, being able to say “you get it” without explaining matters as much as the medicine. You'll find all of me here, and your care is always shaped around your values, not mine.

Questions, answered honestly
The things you're wondering before you reach out.
Nervous about medication? Worried you'll be judged? Not sure anyone will understand? You're in the right place to ask.
Read all FAQsDo I have to stop my medication to get pregnant or breastfeed?
Not necessarily, and you shouldn't stop anything suddenly on your own. Many women can continue or adjust treatment safely through pregnancy and breastfeeding. We review the evidence for your specific medication, weigh the risks of treating against the very real risks of untreated illness, and decide together. Often we coordinate with your OB or midwife so everyone is on the same page.
How do I know if medication is right for me?
You don't have to know before you come in. That's what the evaluation is for. We look at your history, your symptoms, what you've already tried, and your risk factors, and we build a plan around you. Sometimes that includes medication. Sometimes it's integrative support, therapy referrals, or treating a medical issue that was driving your symptoms all along.
Will I be on medication forever?
It depends on the diagnosis. Some people take medication for a season and taper off when they're ready. Some conditions are best managed long-term. Either way, we start low, go slow, and reassess together at every visit. You'll never be rushed into anything or left on something without a reason.
Do you take my insurance? What if I'm paying out of pocket?
We're in network with Aetna (including the State Health Plan), Blue Cross & Blue Shield (including Blue Home), Cigna, UnitedHealthcare/Optum, and Carolina Complete Health (NC Medicaid). Our care coordinator verifies your benefits before your first visit, so there are no surprises. Self-pay is $325 for the initial evaluation and $185 for follow-ups. We don't offer a sliding scale at this time.
Do you see men? Is everything online?
Yes and mostly. Our specialty is women's mental health, and we welcome men and partners too, including dads with postpartum depression. Most visits happen by secure video anywhere in North Carolina. If you'd rather meet in person, we see patients at our Clayton office.
